Trinity School
Parents/Guardians are required to complete a Medical Form prior to their child's
arrival, giving details of any medical history, the dates of childhood
immunisations, and also information about sleeping, dietary requirements, dental
care and any emotional needs. Consent forms for treatment in school and for
anaesthetic in the event of an emergency are also required. All medical
information and consents are updated annually.
